If you live in an area that is not covered by the Network Access Plan (NAP) Plan, or if you are a Medicare-eligible participant on Long-Term Disability (or a Medicare-eligible dependent of a participant on Long-Term Disability), you are eligible for the Comprehensive Access Plan (CAP). The CAP Plan is a fee-for-service plan administered by Anthem Blue Cross on behalf of Anthem Blue Cross Life & Health Insurance Company (Anthem Blue Cross Life & Health). The Claims Administrator is referred to as "Anthem Blue Cross" in this document.
