Transportation and Lodging
  • The Authorization Program will assist the patient and family with travel and lodging arrangements associated with transplant procedures, bariatric surgery or transgender surgery. Expenses for travel and lodging are based on IRS guidelines and include the following: patient and companion transportation, such as airfare, train, or bus fares; hotel or apartment rentals; gas and car rentals; and taxes on covered expenses.
  • Meals do not qualify for reimbursement, per IRS regulations.
For Organ Transplants
  • Travel and lodging expenses are available for transplant patients only if the transplant recipient resides more than 75 miles from the Center of Medical Excellence (CME) facility or a pre-authorized participating kidney or cornea transplant facility.
  • Transplant travel expense for a pre-authorized, specified transplant, including for purposes of evaluation prior to the transplant and post-discharge follow-up
  • If the transplant patient is a covered dependent minor child, the transportation expenses of up to two companions will be covered.
  • There is a combined overall lifetime maximum of $10, per covered person for all transportation and lodging expenses incurred by the transplant recipient and companion(s) and reimbursed under this Plan in connection with all transplant procedures, per IRS regulations.
For Bariatric or Transgender Surgery
  • Travel and lodging expenses are available for bariatric or transgender surgery patients only if the patient resides more than 50 miles from the Center of Medical Excellence (CME) facility or a pre-authorized participating facility performing the surgery.
  • There is a combined overall lifetime maximum of $3,000 per covered person for all transportation and lodging expenses incurred by the patient and companion(s) and reimbursed under this Plan in connection with bariatric or transgender procedures.
